Student Registration

To apply for SSKids Academy, Download the Enrollment Package below and upload the completed forms.

Requirements for Children:

Students must at least the age of 5 years old and adequately potty trained to register for the SSKids Academy program.

Required documents for Admission: 

• Admission Application checklist. 
• Admission Application. 
• Florida Certification of Immunization (DH 680)/ or Immunization Exemption form
• School Entry Health Exam (DH 3040) 
• Intake/Medical History Form
• IEP 
• ABA Assessments
